I had two minor problems:
It turns out (arrghh!) that MH will only put a message-id line in the header if you ask nicely. Grumble grumble. You ask nicely by including "-msgid" as an argument to send.
The "wrong" hostname was the fully qualified domain name of my workstation.
Now:
It turns out (Aaarrgghh!!) that MH was always using the workstation's fully qualified domain name, and sendmail had been "fixing it up".
Thank you, sendmail.
MH will use the correct name if you specify it as the "localname" entry in its mtstailor file.
